Exos contains exemestane, a powerful steroidal aromatase inhibitor (AI) classified as a “suicidal” inhibitor. This means it works by permanently binding to and deactivating the aromatase enzyme responsible for converting androgens into estrogen. At a typical dose of 25 mg per day, exemestane can significantly reduce circulating estrogen levels.

Unlike non-steroidal aromatase inhibitors such as anastrozole or letrozole, which temporarily bind to the aromatase enzyme, exemestane permanently disables the enzyme it attaches to. This mechanism reduces the likelihood of estrogen rebound when discontinued, as the body must create new enzymes to restore aromatase activity.
